ABSTRACT

BACKGROUND: Population surveys across the world have examined the impact of the COVID-19 pandemic on mental health. However, few have simultaneously examined independent cross-sectional data with longitudinal data, each of which have different strengths and weaknesses and facilitate the investigation of distinct research questions. This study aimed to investigate psychological distress and life satisfaction during the first and second lockdowns in the state of Victoria, Australia, and the social factors that may be affected by lockdowns and could affect mental health. METHODS: The VicHealth Victorian Coronavirus Wellbeing Impact Study included two 20-min opt-in online panel surveys conducted in May and September 2020 in Victoria, each with a sample of 2000 adults aged 18 + . A two-part study design was used: a repeated cross-sectional study of respondents who participated in Survey One and Survey Two, followed by a longitudinal nested cohort study. The primary exposures were social solidarity, social connectedness and staying connected with family and friends. Using logistic regression modelling, we explored the associations between our exposures and primary outcomes of psychological distress and life satisfaction with and without adjustment for covariates, both cross-sectionally and longitudinally. The results from the multivariable models were summarised using adjusted Odds Ratios (aOR), 95% Confidence Intervals (CI). RESULTS: Cross-sectional results indicated that the percentage of participants with low life satisfaction was significantly higher in the second survey sample (53%) compared to the first (47%). The percentage of participants with high psychological distress was higher but not significantly different between the two survey samples (14% first survey vs 16% second survey). Longitudinal study results indicated that lower social connectedness was significantly associated with higher psychological distress (aOR:3.3; 95% CI: 1.3-8.4) and lower life satisfaction (aOR:0.2; 95% CI: 0.1-0.4). Younger adults had higher psychological distress compared to older adults (aOR:6.8; 95% CI:1.5-31.1). Unemployment at the time of the first survey was significantly associated with lower life satisfaction at the second survey (aOR:0.5; 95% CI: 0.3-0.9). CONCLUSION: This study supports the findings of other international studies. It also highlights the need to promote increased social connection and maintain it at times of isolation and separation, particularly amongst younger adults.

ABSTRACT

In response to the COVID-19 pandemic, elite sport leagues implemented hubs, or 'bubbles', which restricted athletes' movements and social interactions in order to minimise the risk of athlete infection and allow competitions to continue. This was a new way of working and living for elite athletes and there was a dearth of literature on this topic. The main objective of the study was to investigate the impacts of the hub model on athletes over time, and what job demands and resources existed for athletes through the application of Demerouti et al. (2001) Job Demands-Resources Model. Multiple sequential semi-structured interviews were conducted with Women's National Basketball League (WNBL) athletes during the 2020 season, which was held entirely in a hub in North Queensland, Australia. The key job demands in an elite sport hub identified were the volume of work, simultaneous overload and underload, and nature of work in the hub. The key resources that emerged include recovery services, control and player agency, and constructive social relations. Despite the presence of job resources, which work to counteract, or buffer job demands in order to reduce work stress and improve motivation, they were found to be insufficient for athletes and inequitably distributed between clubs. The intensity of the hub model also amplified demands present in all WNBL seasons. This research is therefore useful for planning of future elite sport leagues to improve the type and amount of resources available to athletes, thereby improving athlete wellbeing and performance both within and outside a hub model.

ABSTRACT

By January 2022 over ten billion doses of COVID-19 vaccines had been administered worldwide. Concerns about COVID-19 vaccine-associated thrombosis arose after the characterization of a rare prothrombotic condition associated with adenoviral vector-based COVID-19 vaccines known as vaccine-induced immune thrombotic thrombocytopenia (VITT). Although mRNA COVID-19 vaccines have not been linked to VITT, concerns about thrombosis after vaccination persist despite safety data from hundreds of millions of recipients of mRNA COVID-19 vaccines. With widespread vaccination some VTE will occur shortly after vaccination by chance alone because VTE is a common condition that affects 1 to 2 in 1000 persons each year. Detailed analysis is required to determine whether these VTE events are coincidental or associated when they occur in close proximity to mRNA vaccine administration. This paper will review what is currently known about rates of VTE after mRNA vaccination in adults, discuss the reasons why uncertainty on this topic persists, and briefly review the implications of these findings for clinical practice and health policy.

ABSTRACT

OBJECTIVE: To identify the challenges adult community sport participants anticipate when returning to sport in Victoria, Australia, post a COVID-19 shutdown. METHODS: Using online concept mapping, participants brainstormed challenges to returning to community sport, sorted them into groups and rated them for impact and ability/capacity to overcome. Analysis included multidimensional scaling and hierarchical cluster analysis. RESULTS: Forty-five community sport participants representing 24 sports identified 69 unique challenges to returning to sport. Eight clusters/questions participants need answered emerged from the sorting data (mean cluster impact and ability/capacity rating out of 5): Will we have enough participants? (3.32, 2.89); How do we stay safe? (3.31, 3.35); How will our sport change? (3.17, 2.85); How can we stay together? (3.15, 3.01); Will I be physically ready? (3.15, 3.05); What about the money? (2.86, 2.53); What about me? (2.65, 3.13); and What about the facilities? (2.49, 2.45). CONCLUSIONS: Participants perceived paradoxical challenges to returning to sport after COVID-19 shutdown, which revolved around staying safe, staying connected and accessing meaningful sport activities. Implications for public health: Sport organisations and public health practitioners should address the participant-centred challenges identified in this study to maximise the public health benefits of participants returning to community sport.

ABSTRACT

Venous thromboembolism (VTE) is the third most common cause of vascular mortality worldwide and comprises deep-vein thrombosis (DVT) and pulmonary embolism (PE). In this review, we discuss how an understanding of VTE epidemiology and the results of thromboprophylaxis trials have shaped the current approach to VTE prevention. We will discuss modern thromboprophylaxis as it pertains to genetic risk factors, exogenous hormonal therapies, pregnancy, surgery, medical hospitalization, cancer, and what is known thus far about VTE in COVID-19 infection.
